Without a great screening tool for blunt cerebrovascular injury we are at risk for missing, or overimaging children with blunt trauma. While we don't have great screening tools, review of the literature does show which physical exam findings are most statistically significant for having a BCVI. Though no single risk factor was found to be statistically significant in every study, the risk factors most found to be statistically significant for BCVI were cervical spine fracture (10/16 studies), skull fracture (9/16), maxillofacial fractures (7/16), depressed GCS score (5/16), and stroke (5/16).
Another finding that should also be considered is T1-3 rib fractures. Also of note is that hanging and clothes-line injuries are likely to not have been included in the study and still have high suspicion for these injuries in these populations.
